Patient Rights 

• You have the right to be treated with courtesy, dignity and respect. 
• It is your responsibility to extend the same courtesy to the practice team. 
• You have the right to be seen urgently if necessary. 
• It is your responsibility not to abuse the appointment system. 
• You have the right to be advised if you will be kept waiting for more than 20 minutes past your appointment time. 

Patient Responsibilities 

• It is your responsibility to cancel appointments that you are unable to keep.
• You have the right to request a home visit. 
• It is your responsibility not to request a home visit unless it is strictly necessary. (The Doctor has the right and responsibility to the final decision on whether a home visit will be made).
